De fait, la fibrovascularisation \u00ab\u00a0int\u00e8gre\u00a0\u00bb davantage l&rsquo;implant dans l&rsquo;organisme, diminuant de mani\u00e8re significative la probabilit\u00e9 d&rsquo;infection ou de rejet.\u00a0La HA est un d\u00e9riv\u00e9 du corail constitu\u00e9 d&rsquo;hydroxyde de calcium et phosphate, substance d\u00e9j\u00e0 pr\u00e9sence dans le corps humain.<\/p>\n<p>&nbsp;<\/p>\n<p>Les implants synth\u00e9tiques en HA arrivent actuellement \u00e0 la troisi\u00e8me g\u00e9n\u00e9ration. L\u2019implant HA synth\u00e9tique FCI3 poss\u00e8de une composition chimique identique \u00e0 celle du Bio-Eye, m\u00eame si la microscopie \u00e9lectronique \u00e0 balayage a d\u00e9cel\u00e9 une moindre uniformit\u00e9 et interconnectivit\u00e9 des pores, ainsi que la pr\u00e9sence d&rsquo;alv\u00e9oles aveugles. Le prix avantageur de l\u2019implant FCI3 synth\u00e9tique lui a gagn\u00e9 en popularit\u00e9 dans de nombreuses r\u00e9gions du monde au cours des 10 derni\u00e8res ann\u00e9es, n\u00e9anmoins les probl\u00e8mes et complications qui y sont associ\u00e9s sont semblables \u00e0 ceux du Bio-Eye. D&rsquo;autres typologies d&rsquo;implant en HA sont utilis\u00e9es dans le monde entier, y compris le HA chinois et les implants br\u00e9siliens. Bien que moins co\u00fbteux que le Bio-Eye, ces implants ont montr\u00e9 des impuret\u00e9s ou une structure poreuse m\u00e9diocre qui offrent peut d&rsquo;avantages par rapport \u00e0 d&rsquo;autres mod\u00e8les disponibles.<\/p>\n<p>&nbsp;<\/p>\n<p>La plupart des applications HA et autres c\u00e9ramiques de chirurgie plastique et reconstructrice concernent la zone cranio-faciale. Elles sont g\u00e9n\u00e9ralement utilis\u00e9es dans le cadre de la reconstruction provisoire de segments squelettiques pour patients oncologiques \u00e0 l&rsquo;esp\u00e9rance de vie limit\u00e9e, en cas de suppl\u00e9ments th\u00e9rapeutiques (RT) ou dans les r\u00e9cessions \u00e0 but palliatif. Pour les d\u00e9fauts de petite taille, la HA est un bon remplisseur et favorise, par ses propri\u00e9t\u00e9s osth\u00e9o-conductrices, la revascularisation et r\u00e9-ossification progressive de la zone trait\u00e9e. L\u2019HA est employ\u00e9e \u00e9galement comme mat\u00e9riau de camouflage de d\u00e9fauts maxillo-faciaux cons\u00e9cutifs \u00e0 des traumatismes ou tumeurs.<\/p>\n<p>&nbsp;<\/p>\n<p>L&rsquo;implant est surtout utilis\u00e9 dans l&rsquo;\u00e9nucl\u00e9ation, positionn\u00e9 \u00e0 l&rsquo;int\u00e9rieur de la capsule de Tenon alors qu&rsquo;il est plus rare de l&rsquo;utiliser dans l&rsquo;\u00e9visc\u00e9ration. La HA n&rsquo;est pas appropri\u00e9e pour l&rsquo;ancrage direct des muscles et ce type d&rsquo;intervention n\u00e9cessite toujours un rev\u00eatement en mati\u00e8re synth\u00e9tique, autologue ou scl\u00e8re de donneur ; elle est en outre difficile \u00e0 mouler car sa tr\u00e8s forte rugosit\u00e9 provoque un traumatisme direct des tissus avec un risque d&rsquo;expulsion. L\u2019HA dans sa forme dense est difficile \u00e0 modeler et sujette \u00e0 migration et extrusion. Les formes micro et macroporeuses aujourd&rsquo;hui disponibles sont beaucoup plus s\u00fbres et ont de meilleures capacit\u00e9s osth\u00e9oconductrices. L\u2019HA\u00a0ne supporte pas de charge et est sujette \u00e0 fracture et rupture compl\u00e8te.<\/p>\n<p>&nbsp;<\/p>\n<p>Dans l&rsquo;intervention d&rsquo;\u00e9nucl\u00e9ation, afin de faciliter l&rsquo;entra\u00eenement de la proth\u00e8se, l&rsquo;implant de HA a \u00e9galement \u00e9t\u00e9 dot\u00e9 d&rsquo;un pivot en titane qui d\u00e9passe de 2 \u00e0 3 mm de la capsule de Tenon et de la conjonctive.
