Like cleansing, decontamination removes most of pathogenic microorganisms making the handling of the above-mentioned items safer during disinfection and sterilization steps. It is performed before cleansing both surgical instruments and patients\u2019 prostheses and respectively after their usage and removal. By decontamination we can:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li>Prevent the spread of infections due to the contact between these objects on one side and environmental surfaces and patients on the other.<\/li>\n<li>Remove visible dirt.<\/li>\n<li>Remove invisible dirt \u2013 carried by microorganisms.<\/li>\n<li>Make all surfaces ready for sterilization and disinfection steps.<\/li>\n<li>Make any further step safe for our personnel.<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Decontamination by peracetic acid (PAA)<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p>Peracetic acid is used in hospitals and both medical and dental offices for high-level decontamination and disinfection of medical devices. It is obtained by diluting a specific water-soluble powder, which generates a chemical-physical balance that leads to a high germicidal active ingredient known as active oxygen. PAA gets thermodynamically unstable in aqueous solution because of:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li><strong>PH<\/strong> increase.<\/li>\n<li><strong>Temperature<\/strong> rise.<\/li>\n<li><strong>Metal ions<\/strong> presence.<\/li>\n<li>Raw materials <strong>impurity<\/strong>.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Once dissolved in water, most of the powder sits at the bottom, so that the active ingredient can continuously circulate keeping a constant concentration level in the solution. This process generates a heterogeneous balance among different components with various functions:<\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li><strong>Sodium perborate<\/strong>, which frees an oxidizing reagent &#8211; the hydrogen peroxide.<\/li>\n<li><strong>Tetraacetylethylenediamine<\/strong> (TAED), which acts as sodium perborate activator and as substrate of acetyl groups for the production of PAA.<\/li>\n<li><strong>Protease, lipase and amylase enzymes<\/strong>, which break organic macromolecular substances of a protein, lipid and glucose nature.<\/li>\n<li><strong>Stabilizers, chelants<\/strong> <strong>and sequestrants<\/strong>, which remove metal ions traces that induce peroxides degradation.<\/li>\n<li><strong>Anionic surfactants<\/strong>, which act as stabilizers and detergents.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p>On the basis of this formula, PAA can be used as:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li>Broad-spectrum and fast-effectiveness disinfectant.<\/li>\n<li>Descaler of protein, lipid and glucose macromolecular material deposited on medical and surgical devices.<\/li>\n<li>Cleanser.<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Scope of use<\/strong><\/p>\n<p><strong>\u00a0<\/strong><\/p>\n<p><strong>First decontamination with simultaneous cleansing<\/strong> of surgical instruments and medical devices, according to D.M. 28 September 1990 art. 2 &#8211; \u201cAfter use and right before dismantling and sterilization steps, reusable devices must be immediately immersed in an anti-HIV chemical disinfectant\u201c.<\/p>\n<p><strong>Intermediate-level disinfection<\/strong> with simultaneous cleansing of surgical instruments and medical devices, paying specific attention to tubercular (TB) activity.<\/p>\n<p><strong>High-level disinfection<\/strong> with simultaneous cleansing of surgical instruments and medical devices by cold chemical sterilization upon validation.[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Mechanism of action<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p>PAA active ingredient acts on lipid membranes, DNA and other cell life essential components, as it interrupts chemiosmotic function of lipoprotein cytoplasmic membranes and transfer into cells, taking advantage of shifts or breaks in cell walls. Its protein denaturant feature may explain its sporicidal action.[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Germicidal activity<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p>This kind of peracetic acid, generated\u00a0 by the release of hydrogen peroxide from the reaction between sodium perborate and tetraacetylethylenediamine (TAED) dissolved in water, has a high biocidal activity towards <strong>spores<\/strong>, <strong>viruses<\/strong> &#8211; The occurrence of this kind of infections is due to several factors: as an example, the presence of people affected by potentially transmissible diseases, or the performance of those treatments that favor both the spread of infectious agents and the resistance of some other particularly virulent microorganisms.[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Treatment rooms<\/strong><\/p>\n<p>Treatment rooms are particularly vulnerable to infections because of both the endless passing of patients, visitors and medical staff and the presence of several potentially contaminating biological fluids. Our treatment rooms get disinfected and decontaminated at every patients access with a specific product, which reduces microbial load below attention level and minimizes the risk of the outbreak of the above-mentioned infections.[\/vc_column_text][vc_separator color=&#8221;turquoise&#8221; border_width=&#8221;2&#8243;][vc_column_text]<strong>Disinfection by alcohol solution and chlorhexidine <\/strong><\/p>\n<p>The product we use to disinfect our treatment rooms is <strong>alcohol<\/strong> and <strong>chlorhexidine<\/strong>-based. Disinfection occurs before patients enter treatment rooms by directly spraying the product on:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li>Patient <strong>chairs<\/strong>.<\/li>\n<li>Operating <strong>tables<\/strong>.<\/li>\n<li><strong>Water fittings <\/strong>and<strong> sinks<\/strong>.<\/li>\n<li>Door <strong>handles<\/strong>.<\/li>\n<li><strong>Telephones<\/strong>.<\/li>\n<\/ul>\n<p><strong>This product contains:<\/strong><\/p>\n<ul>\n<li>Benzalkonium chloride, 0,040 grams.<\/li>\n<li>Didecyldimethylammonium chloride, 0,060 grams.<\/li>\n<li>Chlorhexidine digluconate, 0,050 grams.<\/li>\n<li>Isopropyl alcohol (Ph. Eur.) 26.146.<\/li>\n<li>Ethyl alcohol (Ph. Eur.) 30.020.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Thus, it is a <strong>hydro-alcoholic disinfectant solution<\/strong> composed of different biocides.<\/p>\n<p>As for germicidal activity,<strong> benzalkonium chloride<\/strong> is the most powerful among all quaternary ammonium salts. <strong>Didecyldimethylammonium<\/strong> chloride is a quaternary ammonium salt too, but with a slightly different spectrum germicidal activity. <strong>Chlorhexidine <\/strong>digluconate is a cationic biguanide, soluble in both water and alcohol. In general, the epidermal layer of the skin is neither smooth nor impermeable, so microbial organisms can lie both above and in it occupying follicles, sebaceous glands and sweat glands. Very effective against both gram-positive and gram-negative bacteria, it combines fast disinfection with deep cleansing.
